Effective 2/1/18
Effective February 1st the Department of Defense will increase TRICARE copays for prescription drugs. For home delivery Generic formulary drugs will increase from $0 to $7. Brand-name formulary drugs will increase from $20 to $24 and Non-formulary drugs will increase from $49 to $53. Please be sure the pharmacy has a method of payment on file to avoid delays in your prescription order. See the chart on the Prescriptions and Pharmacies page.


Effective 6/1/17
Nexium and generic esomeprazole magnesium will require prior authorization for all new and current US Family Health Plan members and will change to a Tier 3 copay.  

Once prior authorization is approved, the copay will increase from $0 to $49 at MPHC on-site pharmacies and mail order pharmacy for up to 90-day supply.

The copay will increase from $10 to $50 at retail pharmacies for up to 30-day supply.

Please talk to your provider about switching to one of the preferred: omeprazole (Prilosec®), rabeprazole (Aciphex®) or pantoprazole (Protonix®).
These preferred generics are available at $0 copay at MPHC on-site pharmacies and mail order pharmacy for up to 90-day supply and $10 at retail pharmacies for up to a 30-day supply.

The US Family Health Plan at Martin’s Point will follow the recommendations of the Department of Defense and the TRICARE Formulary Committee and their decision that the effective date will be no later than June 28, 2017.
